Aesthetic icon of femininity and weapon of seduction. The buttocks are often a delight for men and a cross for women: the most seductive symbol of female beauty but also the cause of many insecurities and discomforts. What woman would not like to adorn her body with a Brazilian or mandolin-shaped bottom, with high, small or large buttocks, depending on taste and preference?
Quite apart from a certain obsession that some women (and also many men) have with the beauty of the buttocks, it has to be said that the so-called ‘B-side’ is objectively the element that most influences the design of the female silhouette. And when one is unable to establish a relationship of peaceful acceptance with this area of one’s figure, one can remedy this by resorting to gluteoplasty surgery.
Aesthetic defects and solutions
For reasons of genetic structure or due to the action of gravity with the passing of the years or excessive weight gain (often as a result of pregnancy), the buttocks inevitably have to deal with problems of loss of tone, soft tissue, adiposity or cellulite. Lifting the buttocks, rounding them, increasing or reducing their volume, are the requests most frequently made to the cosmetic surgeon. Curves at the right point, in fact, restore to the physique those proportions that make it harmonious and seductive and encourage a healthy and serene living with oneself.
The imperfections affecting the buttocks concern:
Excess fat and skin; Retractions from cellulite
to be corrected with superficial liposuction (combined, if deemed appropriate, with lipofilling to round and reshape the contours and Subcision of adhesions)
Stretch marks
to be combated with the use of lasers

With Gluteoplasty it is possible to increase the volume of the buttocks, changing their shape and size in cases where they are sagging and relaxed due to weight loss and/or ageing.
The volume increase can be performed by means of:
-Implantation of macromolecular hyaluronic acid (Hyacorp TM)
-Insertion of adipose tissue taken from the patient’s own body (lipofilling) through osmotic filtration with Revolve- Allergan
-use of gluteal silicon implants
Specialist examination
The surgeon must assess the patient’s build and illustrate the modifications that the surgical technique allows on a case-by-case basis. Regarding the problem of “flat buttocks”, it is necessary to supplement and perfect the missing volume by grafting. The ideal correction as envisaged by the BBL (brazilian buttock lift) involves taking fat tissue from other areas of the patient’s body and reinserting it into the part of the buttocks that is hollowed out. It can happen, however, that the amount of fat taken is not sufficient for filling and therefore it is necessary to resort to gluteoplasty with implants. The shape of the prosthesis is also chosen according to certain characteristics of the patient, such as the pelvis or the original appearance of the buttocks.
If the problem concerns an overly abundant buttocks, the patient’s shape must be assessed in the entirety of the body, evaluating the abdomen-thighs-buttocks context and intervening with an integrated approach.
At the end of the examination and after showing the patient photographs of cases similar to his own, Dr Tommaso Savoia will propose the most appropriate technique, illustrating the areas that aesthetically are most worthy of surgery.
Preparation for surgery
In order to gather all the information about the patient’s state of health that will rule out diseases of various kinds, the doctor will prescribe the necessary clinical tests, including a complete blood test, a urine test and an electrocardiogram. If the tests are found to be normal, the patient can be operated on. The plastic surgeon will also be informed by the patient about any intolerance to antibiotics and medication, and about the treatment he or she is taking. It is advisable not to take aspirin or medicines containing acetylsalicylic acid for about two weeks before surgery and two weeks afterwards. Aspirin can cause bleeding and thus increase the risk of complications.
Surgery
Both in the case of lipofilling and in the case of using implants, it is desirable for the operation to take place under general anaesthesia or with more or less deep sedation.
Gluteal lipofilling-BBL
The first option, as already mentioned, consists in an autologous transplantation, i.e. in the reuse of fat taken from the patient’s body by means of Tulip cannulas with microholes and diameters not exceeding 2 mm and its osmotic washing with Revolve advanced adipose system ( http://hcp.revolvefatgrafting.com/revolve-system) and reintroduced into the gluteal region. The closed circuit allows the fat to be suctioned and washed at the same time without any risk of contamination and with a maximum take-up of up to 70%. In the preoperative phase Dr Savoia decides the selection of the fat harvest areas generally trochanters, knees, hips and lumbar area and ankles and modelling with liposculpture the rest of the body while preparing the fat that will then be implanted with a multilayer technique called ‘spaghetti technique’ to allow maximum adhesion of the adipocytes to the new tissue. After an incision is made, the adipose tissue is sucked through thin cannulas.
Lipofilling buttocks duration
With the latest fat treatment techniques, the percentage of reabsorption of the transplanted adipose tissue will be very low. It is however necessary to assume an increase in volume on each side of at least 250-350 cc of fat tissue, so it will not be a viable option in those who are already very thin.
Gluteoplasty with implants
While the amount of autologous material (removed from the thighs, hips and/or abdomen) does not cause allergies and guarantees a completely natural result, it may not be sufficient to fill out the hollowed-out areas. This is especially the case for thin subjects who lack sufficient amounts of body fat.
In the latter case, the option of implanting prostheses will be pursued, provided that the individuals have elastic, firm skin and a good level of muscle tone. The procedure allows a volume of about 400 cc to be implanted per buttock. The procedure involves making an incision of about 5/6 cm in the intergluteal groove, so that the scar remains hidden between the buttocks. In under muscular space the prosthesis will be inserted, which generally has a smooth surface and is made of the latest generation of silicone. It is also convenient to use drains that will only be left in place for a few days.
Hyaluronic acid buttocks
As a third possibility, remodelling of the buttocks can also be achieved with the use of hyaluronic acid in 10 ml ampoules (hyacorp; https://www.hyacorp.fr/en/body/buttocks/) injected through a cannula deep into the tissue. In this case, the surgeon will operate under local anaesthesia and no hospitalisation will be necessary. It will be necessary to assume at least 3-4 ampoules per side in order to have adequate visible remodelling.

The duration of each procedure may vary depending on the procedure applied and the patient’s condition. In general, complications related to the procedure include infections (to be prevented with antibiotic treatment), haematomas, asymmetries and irregularities of the skin profile.
Post-operative
Absolute rest is prescribed in the 48 hours following the operation. It is also recommended to sleep on one’s back for about two weeks, and from the second day after the operation it is possible to sit up. From the third day onwards, a calm lifestyle can be resumed,
avoid strenuous activities, saunas, Turkish baths and exposure to the sun. In the first two days after surgery, swelling, bruising and pain may occur in the treated region. After 7-10 days, work activity, if not excessively strenuous, can be resumed. It is also recommended to wear a compression sleeve continuously for about a month.
Costs
As we have illustrated, the methods for obtaining firm and perfect buttocks are numerous, and therefore the price ranges also depend on the surgical procedure used and other factors (type of anaesthesia, admission to day hospital or not, etc.).
As an indication, prices range from 1,500 for fillers to 6,500-8,000 euro for BBL when combined with Vaser Lipo and Argon Plasma to reshape the sagging areas
